Informed Consent

We believe you should have a good idea of what your getting into before you get into definitive treatment.  Our policy is to provide a thorough examination.  Once the examination is completed, we will develope a treatment plan based on the findings of the examination and on your direct imput as to what treatment you would like done and on what your major chief concerns are.  Together, you and I will develope this treatment plan, which we will put in writing, complete with sequence of treatment, cost of each procedure, estimated insurance benefits, and estimate out of pocket costs (portion not covered by insuance).  We will discuss options to our proposed treatment and potential complications for treatment or non treatment.  We will do our best to give you an accurate estimate for your treatment prior to performing any treatment beyond your first examination and x-ray appointment.  Of course there can be changes to treatment as treatment progresses.  When this (change) occurs, we will stop, discuss treatment changes and cost changes with you and you will decide what course of action to take.  We work for you, we are your professional consultant, and ultimately you will decide what treatment to accept and how much you can do.
